Getting a referral
If you need to see the doctor (a psychiatrist) at the Deafness and Mental Health Service, your GP needs to complete the Referral Request Form and you need to complete the Patient Registration and Consent Form.
If you need help from the Deafness and Mental Health Service but don’t need to see the psychiatrist, you just need to complete the Patient Registration and Consent Form.
You or your GP can email the forms to us at deafness_mhs@health.qld.gov.au.
If you need help with a referral, you can email us at deafness_mhs@health.qld.gov.au or text us on 0427 598 076.
If your mental health is very bad, please:
- go to your GP
- call 1300 MH CALL (1300 642 255) via NRS National Relay Service (nrscall.gov.au), or
- go to the emergency department at your local hospital.
